第一确实应该加the 第三句没啥问题，第四也没啥问题。
确定是书上的？哪本书？
第二句啊，没有the很正常啊，不需要特指。
第三句和第一句口语上来说好像是可行的，第三句没有the也没啥奇怪，不过一般year作为可数名词不能单独存在，不加也没有歧义，加上a或者the比较正常。
第四句，两句都没有错，North hospital 属于专有名词不需要加，后面表示强调。
the 这个问题确实需要长时间去习惯

第一句没问题，它表示她处于一排人的两头（第一个），试与下句比较：She is the first in the quene having passed the exam.

第二句：adjectives前面不需要有the，因为
所有的形容词都要注意其顺序。

第三个句子，前面的表示特定的年份和特定的季节；后面的句子表示每年的一个特定的时间。

第四个句子中后边一句的the应该去掉。
